Langzeitfolgen nach Krebs im Kindes- und Jugendalter – Die Nachsorge der von einer Krebserkrankung geheilten Kinder und Jugendlichen

Zusammenfassung

Dank verbesserter Therapiemöglichkeiten haben sich die Langzeitüberlebensraten der Patienten mit einer Krebserkrankung im Kindes- und Jugendalter in den letzten Jahrzehnten deutlich verbessert. Durch die zur Heilung der Krebserkrankung eingesetzten Therapien können jedoch auch viele Jahre nach Therapieende noch Folgeerkrankungen, sogenannte Spätfolgen, auftreten, die zu einer hohen Morbidität und verminderten gesundheitsbezogenen Lebensqualität der Langzeitkrebsüberlebenden führen. Das Auftreten vieler Spätfolgen erfolgt mit einer langen Latenz zum Behandlungsende und wird durch die erhaltene Behandlung und durch individuelle Risikofaktoren der Patienten bestimmt. Dabei können die Spätfolgen unterschiedliche Organe betreffen und von leichten Störungen bis zu lebensbedrohlichen Komplikationen oder Erkrankungen, wie beispielsweise einer neuen Krebserkrankung, reichen. Viele Patienten sind zum Zeitpunkt des Auftretens der Spätfolgen allerdings bereits erwachsen und befinden sich nicht mehr in regelmäßiger pädiatrisch-onkologischer Betreuung. Da das meiste Wissen über Spätfolgen aus den letzten 20 Jahren stammt, sind zudem viele der zuvor erkrankten Patienten nie über ihr Risiko für potenzielle therapieassoziierte Folgeerkrankungen aufgeklärt worden. Dies ist die Aufgabe für die betreuenden Ärzte während der Langzeitnachsorgebetreuung, um mögliche Spätfolgen frühzeitig entdecken und behandeln zu können.
