Breast Reduction/Mastopexy with Superomedial Pedicle

Abstract

The major advantages of the superomedial pedicle breast reduction/mastopexy are superior pole fullness, reliability and safety regarding NAC viability, and more durable results on long-term follow-up with decreased risk of bottoming-out. The authors discuss a modification of the technique that leads to safer and more predictable results. By incorporating the superior component to the medial pedicle, additional blood supply through the lateral thoracic artery to the internal mammary artery perforators is brought, decreasing the chances of NAC suffering. Likewise, additional nerve supply through anterior and lateral cutaneous branches of the intercostal nerves increases the chance of preserving NAC sensibility.
