Zusammenfassung
Die Nachsorge von Patienten nach Nierentransplantation findet in den Transplantationszentren und bei den niedergelassenen Nephrologen und Kindernephrologen statt. Eine flächendeckende sektorenübergreifende Versorgungsstruktur für nierentransplantierte Patienten existiert bis heute in Deutschland nicht. Somit ist die Versorgung dieser Patientengruppe optimierungsbedürftig. Grundsätzlich vermeidbar sind die häufig zu beobachtende Non-Adhärenz sowie das hohe kardiovaskuläre Risikoprofil dieses Patientenkollektivs, die hauptursächlich für frühzeitige Transplantatverluste sind. Ziel des vom Innovationsfonds geförderten Nachsorgeprojekts NTx360° ist die Verbesserung des Transplantatüberlebens und der Lebensqualität sowie eine Verbesserung der Wirtschaftlichkeit der Versorgung nierentransplantierter Patienten. Die Nachsorgequalität soll durch eHealth-Elemente, ein Adhärenz-Coaching, individuell zugeschnittene Trainingstherapien, ein intensiviertes Fallmanagement und eine sektorenübergreifend nutzbare elektronische Fallakte optimiert werden. Die Ergebnisse von NTx360° werden mit Vergleichsdaten von Krankenkassen extern durch das IGES-Institut evaluiert. Ziel ist es, positiv evaluierte Elemente des Projekts NTx360° in die Regelversorgung zu überführen.
