Zusammenfassung
Hohe Prävalenzraten resistenter Staphylokokken (MRSA) und Enterokokken (VRE), eine bedrohliche Ausweitung der Resistenzentwicklung bei gramnegativen Stäbchenbakterien und die Selektion von Clostridium difficile bei gleichzeitig deutlichem Rückgang in der Entwicklung neuer Antibiotika haben weltweit den rationalen Einsatz der vorhandenen Antibiotika in den Focus internationaler und nationaler Kampagnen gestellt. Die Implementierung von Antibiotic-Stewardship-Programmen dient dabei dem Ziel, durch einen rationalen Einsatz der vorhandenen Antibiotika ihre Wirksamkeit zu erhalten. Wesentliches Ziel einer Therapie mit Antibiotika ist die Heilung eines individuellen, an einer bakteriellen Infektion erkrankten Patienten. Das Erreichen bester klinischer Behandlungsergebnisse kann nur unter Beachtung einer Minimierung von Toxizität, Selektion pathogener Erreger und Resistenzentwicklung erzielt werden. In diesem Kapitel werden die Grundlagen einer rationalen Antibiotikatherapie dargestellt.
