Zusammenfassung

Eine direkte Darstellung der Pfortader ist möglich mittels Direktpunktion unter Computertomographie‑, Ultraschall- oder Durchleuchtungskontrolle.

Indirekte Portographie über eine Mesenterikographie:

  • Punktion der (meist) rechten A. femoralis und Einlage einer kurzen (10–12 cm) 4‑F‑Schleuse.

  • Einhängen eines 4‑F‑Sidewinder‑C1‑ (nach seiner Konfiguration in der A. subclavia sinistra) oder eines 4‑F‑Cobra-C2‑Katheters in die A. mesenterica superior (AMS)

  • Durchführen einer Serie mittels Injektorpumpe mit 40–50 ml Kontrastmittel (KM): 5–6 ml/s; 2 Bilder/s, dann Umschalten auf 0,5 Bilder/s in der Pfortaderphase (darauf achten, dass die Serie zeitlich ausreichend lang läuft, damit die Pfortaderphase erreicht wird)

Die Indikationsstellung erfolgt interdisziplinär (meist mit Chirurgie und Gastroenterologie) bei symptomatischen Patienten (es handelt sich dabei jeweils um eine Einzelfallentscheidung). Typische Symptome sind schwere Bauchschmerzen, die bis zu einem akuten Abdomen reichen können.

Bei einem akuten Verschluss aller Etagen (intrahepatische Pfortader bis inkl. Mesenterialvenen) ohne Nachweis einer bereits eingetretenen, kavernösen Transformation stellen wir die Indikation aggressiver (◘ Abb. 17.1).
