Zusammenfassung
Eine direkte Pfortaderdarstellung ist möglich mittels Direktpunktion unter Computertomografie-, Ultraschall- oder Durchleuchtungskontrolle. Eine indirekte Portografie erfolgt über eine Mesenterikografie. Die Indikation zur Pfortaderlyse erfolgt interdisziplinär bei symptomatischen Patienten (Einzelfallentscheidung). Typische Symptome sind schwere Bauchschmerzen, die bis zu einem akuten Abdomen reichen können. Bei einem akuten Verschluss aller Etagen (intrahepatische Pfortader bis inkl. Mesenterialvenen) ohne Nachweis einer bereits eingetretenen, kavernösen Transformation stellen wir die Indikation aggressiver. Besteht in der Schnittbildgebung bereits der Verdacht auf eine venöse Kongestion des Darms, ziehen wir eine chirurgische Exploration einer erwogenen Pfortaderlyse vor.
