Crises During ECLS

Extracorporeal Membrane Oxygenation for Adults

Abstract

Expeditious recognition and effective intervention during crises are essential to the successful management of patients receiving extracorporeal life support (ECLS). Crises during ECLS can be partitioned into those originating in the circuit and those originating from the patient. Circuit crises, such as the presence of circuit air, thrombosis, oxygenator failure, pump failure, tubing rupture, heater malfunction, and inadvertent decannulation, and patient crises, such as bleeding, hemolysis, refractory hypoxemia, and shock, may quickly become life-threatening emergencies. Providers must be trained to promptly identify and respond to both common and uncommon complications of ECLS in order to optimize patient safety and improve clinical outcomes.
