Hip Resurfacing Arthroplasty Using the Hueter-Anterior Approach

Abstract

Hip resurfacing arthroplasty remains a viable option for the young active patient with symptomatic osteoarthritis, and arguably may be superior to total hip arthroplasty in some parameters. Historically, one of the main challenges of performing hip resurfacing was exposure of the femoral head, with the attendant risk of injury to its blood supply. Significant soft tissue dissection is usually required to expose the femoral head through posterior or anterolateral approaches. The Hueter-anterior approach using an orthopaedic positioning table avoids risk to vascularity of the head, while respecting the soft tissues around the hip. This approach is well established for primary total hip arthroplasty, with excellent function and a very low dislocation rate. This chapter provides an overview of the key technical considerations in performing resurfacing arthroplasty through the Hueter-anterior approach and presents the senior author’s personal experience with this technique.
