Therapy of Sleep Disorders in Parkinson’s Disease

Abstract

Up to 80% of patients with Parkinson’s disease suffer from a wide range of sleep disorders in all stages of the disease. While restless legs syndrome as one contributor to a disturbed night sleep can be present at any stage in the disease, sleep disturbances in the advanced stages of Parkinson’s disease are usually linked to motor impairment during the night. In late stages of the disease psychiatric symptoms, with nocturnal hallucinations and disorientation due to cognitive decline, often contribute to disturbance of night sleep. In contrast, rapid eye movement sleep behavioral disorder can antedate the onset of motor symptoms in Parkinson’s disease and is considered a robust marker and thereby prodromal symptom of an emerging neurodegeneration due to alphasynuclein accumulation. The heterogeneity in pathophysiology and clinical manifestation makes it impossible to recommend one uniform therapy strategy for sleep disorders associated with Parkinson’s disease. This chapter offers a comprehensive approach to diagnosis and management of the different sleep disorders in Parkinson’s disease.
