Laparoscopic One Anastomosis Gastric Bypass/Mini Gastric Bypass: Revisional Surgery for Failure in Weight Loss and Metabolic Outcomes

Abstract

One anastomosis gastric bypass (OAGB) is a growing worldwide-accepted procedure that comprises both gastric restriction and intestinal hypoabsorptive action. The main differences in comparison with regular Roux-en-Y gastric bypass (RYGB) are based on a single gastrointestinal anastomosis in a more distal derivation. Nowadays, OAGB surgery is already the third-most widely used bariatric and metabolic procedure, tending to get closer with RYGB soon.

While protein-calorie malnutrition and biliary reflux seem to be the major reasons for surgical revision after OAGB due to the more distal gastrointestinal derivation leading to a shorter common channel, the occurrence of insufficient weight loss or weight regain is not commonly mentioned in the OAGB medical literature missing a standardized approach in revisional surgery to optimize weight loss after OAGB.

As with any other bariatric procedure, the initial approach should be based on correctly identifying the failure reasons and include an intensive, multidisciplinary counseling. Reoperation will be based on increased restriction by the pouch and gastrointestinal resizing or distalization by transforming in a biliopancreatic derivation model or procedure or just decreasing the length of the common channel.
