Abstract

Liver transplantation is recognized widely as the ultimate treatment for patients with acute or chronic liver failure. Because it is one of the most challenging noncardiac surgical procedures, its success depends on an anesthesiology team with a special set of knowledge and skills, including a thorough understanding of the systemic manifestations of liver failure, donor types, transplant procedures, the three stages of transplantation surgery, and related issues. Special attention should be paid to blood transfusion conservation strategies and the initiation of induction immunosuppression therapy. The goal of this chapter is to describe this basic knowledge and discuss the anesthetic management of liver transplantation recipients. Practical anesthesia management is described according to the three stages of liver transplantation: stage I (pre-anhepatic), stage II (anhepatic), and stage III (neohepatic).
