Abstract

Fistula-in-ano remains a complex disease process. The treatment for anal fistulas has evolved to include a variety of sphincter-preserving modalities. Over the past 2 decades there has been an increased interest in fibrin sealant as a sphincter-sparing modality for the treatment of fistula-in-ano. Fibrin sealant application is easy, safe, moderately effective, repeatable, and associated with early return to normal activity. The use of fibrin sealants in the management of anal fistulas continues to evolve including its use as a delivery vehicle for cytokines and stem cells. This chapter will discuss the biology, rationale, historical aspects, technical details, complications, and extensive literature review of fibrin sealants in the treatment of fistula-in-ano.
