Abstract
In this chapter, I review the implications of the current diagnostic criteria for fetal alcohol spectrum disorders (FASD) in corrections populations where it is often extremely difficult to determine the highly restrictive exposure criterion for a diagnosis of FASD. So little data is available on adults with FASD that it is unclear if the current criteria for FASD are appropriate for use with adults and the elderly. Data demonstrating that the current multidisciplinary model would not be able to scale up to meet the needs of people in prisons with FASD is presented. The current criteria for FASD are not useful for the elderly, and a new phenotype for FASD needs to be developed. Many people with FASD in adult corrections populations meet criteria for a developmental disability, and this will require very large changes in the service delivery system for this population. I discuss the urgent need for early diagnosis to enhance our ability to prevent secondary disabilities and use adverse childhood experiences (ACEs) as an example. The chapter concludes with the recommendation to use screening in prison systems as an initial step in the diagnosis of FASD and as the eligibility criteria for FASD specific services.
