1 Einleitung
Fettstoffwechselstörungen werden aus zwei Gründen therapiert: Erstens zur Senkung des Risikos kardiovaskulärer Erkrankungen und zweitens zur Prävention einer akuten Pankreatitis bei Hypertriglyzeridämie. Wegen des grundsätzlich unterschiedlichen Ansatzes werden beide Indikationen getrennt betrachtet.
2 Therapie von Fettstoffwechselstörungen zur Senkung des Risikos kardiovaskulärer Erkrankungen
Angesichts ihrer nach wie vor hohen Inzidenz und Prävalenz kommen der Primär- und Sekundärprävention kardiovaskulärer Erkrankungen mit einer Lipidtherapie eine außerordentlich hohe Bedeutung zu. Atherogene Lipoproteine sind vor allem LDL, Lipoprotein (a) (Lp[a]) sowie die Restpartikel („remnants“) von Chylomikronen und VLDL. Die HDL-Konzentration ist nicht signifikant therapeutisch beeinflussbar; HDL-Partikeln wird eine atheroprotektive Funktion nachgesagt.
