Zusammenfassung

Die sekundär sklerosierende Cholangitis (SSC) ist eine cholestatische Gallenwegserkrankung, die gekennzeichnet ist durch Inflammation, obliterative Gallenwegsfibrose, biliäre Strikturen und schließlich biliäre Zirrhose. Gerade die SSC nach Low-output-Syndrom oder septischem Schock stellt eine gravierende mittelfristige Komplikation der modernen Intensivtherapie dar. Innerhalb weniger Wochen kann die SCC zu einem zirrhotischen Leberparenchymumbau führen. Die wenigen verfügbaren prophylaktisch-therapeutischen Optionen bestehen in der Therapie der Grunderkrankung, der endoskopischen Extraktion der biliären Casts mit Aspiration von Galle zur mikrobiologischen Diagnostik und Auswahl der antiinfektiven Therapie, sowie – in ausgewählten Fällen – in einer Lebertransplantation.
