Abstract

Plantar plate insufficiency is a common cause of metatarsalgia, and it may progress to toe coronal and sagittal plane malalignment with time. Conservative treatment can rarely delay the progression and several surgical soft tissue balancing, and bony procedures have been described. Plantar plate tenodesis is one of the soft tissue balancing procedures, and it can be performed precisely under arthroscopic assistance. Through the stabilization of the attenuated plantar plate by suturing of the plantar plate with the extensor tendon, it acts as a dynamic repair to correct the flexible claw toe deformity. The technique of the arthroscopic-assisted plantar plate repair has gone through several modifications, and the modified double plantar plate tenodesis technique (the fourth generation), the postoperative care, and the surgical outcome will be described in this chapter.
