Loop ileostomy: A superior diverting stoma in colorectal surgery

Abstract

In a consecutive series of 99 patients, we have evaluated the safety of the loop ileostomy employed for diversion and/or decompression. We have recorded the complications after construction and subsequent closure and any associated difficulties with stoma care have been evaluated. The results indicate that a loop ileostomy, when properly constructed and managed by modern appliances, presents no more problems than a conventional end ileostomy. Closure of a loop ileostomy is associated with few complications. A loop ileostomy appears superior to a transverse colostomy as a temporary diversion of the fecal stream and should be used where possible.

Résumé

Chez 99 sujets qui subirent une iléostomie latérale par extériorisation d'une anse intestinale pour assurer l'évacuation du liquide intestinal ou pour décomprimer l'intestin, les conséquences de l'intervention ont été étudiées. Ainsi ont été colligées les complications secondaires à la constitution de l'iléostomie, à sa fermeture, et aussi les difficultés inhérentes aux soins imposés par l'iléostomie. Les résultats de cette étude indiquent que l'iléostomie latérale sur anse extériorisée, quand elle est bien faite et qu'elle est soumise ultérieurement aux soins adéquats, ne présente pas de problèmes plus difficiles que ceux de l'iléostomie terminale classique. La fermeture de l'iléostomie latérale entraîne peu de complications. L'iléostomie latérale est en fait supérieure à la colostomie transverse pour assurer la dérivation externe du bol fécal et elle doit être établie quand il est possible de la réaliser.

Resumen

Hemos evaluado la seguridad de la ileostomía de asa empleada como medio de desviación y/o descompresión, en una serie de 99 casos. Registramos las complicaciones a partir de la construcción y más tarde de su cierre, así como la evaluación de cualesquiera dificultades relacionadas con el cuidado del estoma. Los resultados indican que la ileostomía de asa, si ha sido debidamente construída y manejada con los modernos aditamentos disponibles, presenta problemas que no son mayores que los de una ileostomía terminal convencional. El cierre de una ileostomía de asa se acompaña de pocas complicaciones. La ileostomía de asa parece ser superior a la colostomía transversa como forma temporal de desviación del torrente fecal y debe ser utilizada en cuanto sea posible.
