Summary
We report a case of pneumonia, caused byBordetella bronchiseptica, in a previously healthy, immunocompetent 37-year-old male patient who had suffered chest injury in a car accident. The patient was admitted to the Intensive Care Unit where endotracheal intubation was performed. Seventy-two hours later he presented with fever associated with pulmonary affection which was diagnosed as right lobar pneumonia. Abundant colonies ofB. bronchiseptica were isolated from the pharyngeal exudate and respiratory secretions, suggesting prior oropharyngeal colonization byB. bronchiseptica, as a result of repeated contact with his dog, with subsequent infection of the lower respiratory tract assisted by the process of intubation. We review different human infections produced byB. bronchiseptica as well as the antibiotic susceptibility studies performed.
Zusammenfassung
Ein vorher gesunder, immunkompetenter 37jähriger Mann entwikkelte nach einem Thoraxtrauma durch Autounfall eine Pneumonie, die durchBordetella bronchiseptica verursacht war. Nach Verlegung auf die Intensivpflegestation wurde die endotracheale Intubation vorgenommen. 72 Stunden später traten Fieber und Zeichen einer Lobärpneumonie rechts auf. Aus Pharyngeal- und Bronchialsekret wurdeB. bronchiseptica in Massen isoliert, was vermuten ließ, daß bereits — möglicherweise durch Kontakt mit dem Hund — eine oropharyngeale Kolonisation mitB. bronchiseptica bestanden hatte und der Erreger durch Intubation in die unteren Luftwege gebracht wurde. Andere Infektionen durchB. bronchiseptica beim Menschen werden dargestellt und die Ergebnisse der Empfindlichkeitstestung mitgeteilt.
