Dieses Kapitel widmet sich dem organisatorischen Aufbau von Palliativstationen, Palliativdiensten und Palliativambulanzen im Krankenhaus, dem Entlass managament und dem organisatorischen Aufbau ambulanter Palliativstrukturen. Es vermittelt einerseits einen Überblick über die Versorgungslandschaft für Palliativpatienten und beleuchtet andererseits deren Interaktion mit schon bestehenden ambulanten und stationären Strukturen. Des Weiteren bietet dieses Kapitel Anregungen und praktisches Know-how für den Aufbau neuer Palliativstrukturen und deren Integration in bestehende Organisationen. Weiterhin geht es um
