The Effect of Radiation on Acellular Dermal Matrix and Capsule Formation in Breast Reconstruction

Abstract

In the United States alone over 95,000 breast reconstructions were performed in 2013 (ASPS. Procedural Statistics, 2013). The two-stage tissue expander reconstruction accounts for over 75 % of these (J Am Coll Surg 217(4):656–664, 2013) and will likely continue to increase in prevalence as the healthcare industry evolves. Acellular dermal matrices (ADM) have become a key component in expander breast reconstruction due to a decrease in expansion time, lack of donor site morbidity, and improved definition of the inferior and lateral breast margins. At the same time, the indications for post-mastectomy radiation are expanding as new data supports an improved safety and efficacy profile (J Clin Oncol 28(14):2396–2403, 2010; Strahlenther Onkol 185(10):656–662, 2009). Therefore, surgeons must understand the effects of radiation on dermal matrices and counsel patients on the safety and complication rates of expander/ADM reconstruction subjected to post-mastectomy radiation.
