Abstract

This chapter concerns three diseases of the gut of unknown aetiology— pernicious anaemia, ulcerative colitis and Crohn’s disease. In each there is mucosal inflammation which, by creating greater permeability to antigen present in the lumen, will stimulate local and systemic immune responses. It is therefore not surprising that many immunological phenomena have been described. Much interest has centred around the possibility that these diseases are caused by responses directed towards ‘self-antigens’, and around the mechanisms by which these responses might arise. These include mucosal inflammation itself which may alter mucosal antigens or reveal those previously sequestered, and the possession of common antigens by mucosa and intestinal micro-organisms. It also seems likely that the ability to mount these responses is partly genetically controlled.
