Summary
Since somatoform disorders were first included in DSM-III, Chinese psychiatrists have been concerned about these disorders. Some reviews and lectures were published in Chinese journals. During 1987–1992, the definition and diagnostic criteria were introduced in ICD-10 and DSM-III-R, and some Chinese doctors took an interest in these disorders and began to carry out clinical studies. However, the classification was not included in the Chinese Classification of Mental Disorder (CCMD) and its second revision (CCMD-II-R). The probable reasons are: (1) Until now the medical administration departments in China only authorize all hospitals to adopt the ICD-9 which does not include somatoform disorder. (2) CCMD-II-R still classifies hysteria as a disease (3) Chinese specialists are in dispute regarding somatoform disorder. Some claim that the definition of this disorder is unclear, its etiology and pathogenesis are unclear, and its symptoms are difficult to differentiate from those of depression, anxiety, hypochondriasis, neurasthenia, and medical problems. The proponents claim that patients with this disorder are commonly seen in general and mental hospitals. Keeping this disorder in the classification system of mental disorders is helpful in clinical treatment. The CCMD-III protocol is now being edited and tested. The disagreements will be discussed.
