Abstract
Amongst the large range of potential etiologies concerning chronic low back pain, with or without temporary sciatica, there is one cause that seldom comes in mind: the discogenic origin of the pain. Since 1978 we have maintained that the overpressure inside the disc and the over-distension of the peridiscal ligamentous coats cause the local pain and are the source of the metameric painful reflexes. As a consequence, the logical treatment would be to decompress the disc by drilling a percutaneous lateral fenestration and a tunnel up to the nucleus for draining nuclear materials.
