Stratégie de dépistage de l’ischémie silencieuse chez le diabétique asymptomatique

Résumé

L’intérêt du dépistage et les moyens à mettre en œuvre pour dépister une coronaropathie chez les diabétiques asymptomatiques de type 2 restent l’objet de controverses. Les cardiopathies ischémiques et l’insuffisance cardiaque sont la première cause de mortalité et de morbidité chez les diabétiques et la prévalence des diabétiques dans la population générale ne cesse d’augmenter, ce qui pose un problème de santé publique. Plusieurs questions restent d’actualité

  • le dépistage de l’ischémie silencieuse doit-ilêtre systématique dans cette population ou doit-ilêtre ciblé en fonction de facteurs de risque associés?;

  • ce dépistage est-il bénéfique pour le pronostic à long terme du patient?;

  • Quel examen, ou quelle stratégie diagnostique, est le plus efficace pour évaluer ces patients asymptomatiques? Nous nous intéresserons ici aux patients diabétiques de type 2 sans cardiopathie ischémique connue.
