It is here, in Chapter 11, that we pull together all of the inform and knowledge that we have gained throughout this entire book to create the all-important “pipeline” consisting of all the key steps that are so critical to the successful launching and running of all DIGMAs and PSMAs (many of which apply equally to CHCCs) implemented within the system. The pipeline presented here is simply an implementation tool for the champion and program coordinator to use for efficiently launching numerous DIGMAs and PSMAs throughout the entire system as easily and rapidly as possible, and without having to reinvent the wheel time and time again. It is important to note that the pipeline that we discuss in this chapter is not to be viewed as static; in fact, you will undoubtedly find it to be constantly evolving and improving over time in your system through continuous process improvement. It is something that the SMA champion and program coordinator will always be updating, streamlining, and modifying as experience is gained and operational issues are addressed. Therefore, the pipeline is best viewed as a work that will always be in progress.
