Transactions of Red Packets in the Hospital

Informal Payments and Regulations in China's Healthcare System

Abstract

This chapter, drawing on empirical findings, examines red packet transactions at clinical level. It analyses the conditions under which doctors take and decline red packets. The findings indicate that patients tend to give red packets to doctors before the treatment and the giving is motivated by a wide range of purposes. Doctors are usually cautious about “cold” offered red packets but are far more receptive to those given through guanxi. Red packets can help patients jump the queue, engage senior doctor, and improve doctors’ attitudes, but are unlikely to have impact on the technical core of medical treatment. Under some circumstances doctors tend to decline red packets, which may cause misinterpretation and lead to clashes between doctors and their patients.
