Samenvatting

Diepe hersenstimulatie (‘deep brain stimulation’, DBS) bij bewegingsstoornissen en psychiatrische aandoeningen kan bij een deel van de patiënten gepaard gaan met veranderingen in de cognitie, de stemming en het gedrag. Deze veranderingen worden door meerdere factoren bepaald, waaronder de persoonlijkheid van de patiënt. Vooralsnog is er geen wetenschappelijke onderbouwing voor een bepaald risicoprofiel. Om het succes van DBS te optimaliseren, is het nodig bij de individuele patiënt de oorzaak van de veranderingen te achterhalen en aan de hand hiervan de behandeling aan te passen. Ook is het belangrijk aandacht te hebben voor het aanpassingsproces dat patiënten na hun operatie doormaken en de veranderingen in rolpatronen binnen het systeem van de patiënt. Ten slotte is het van belang vóór de operatie de verwachte verbetering en de risico’s op bijwerkingen goed in kaart te brengen en deze samen met de patiënt af te wegen, opdat teleurstellingen na de operatie voorkomen kunnen worden. Een goede pre- en postoperatieve inventarisatie van de cognitie, stemming en het gedrag, en preoperatief ook van de persoonlijkheid, is een taak voor de klinisch neuropsycholoog en een voorwaarde voor zorg op maat.
