Samenvatting

Een antimicrobieel scherm moet de patiënt met een verminderde afweer, gegeneraliseerd of lokaal, beschermen tegen infecties. Een dergelijk scherm bij orale problemen of tandheelkundige en mondhygiënische behandelingen kan van levensbelang zijn bij patiënten met ziekten die gepaard gaan met een verminderde afweer of bij patiënten bij wie de verminderde afweer medicamenteus wordt nagestreefd. Beide situaties kunnen zich ook bij één patiënt voordoen.

Patiënten die als gevolg van een verminderde afweer herhaalde infecties hebben, moeten noodgedwongen regelmatig met antimicrobiële middelen worden behandeld. Ter bescherming tegen de ontwikkeling van zowel allergische reacties als tegen resistentie wordt elke indicatie gewikt en gewogen. Het is de medisch specialist vaak bekend of er al allergische reacties zijn opgetreden. Ook zijn er kweken bekend met resistentiepatronen, zodat een gerichte keuze voor een antimicrobieel middel gemaakt kan worden. De wensen van de behandelende arts en de tandarts met betrekking tot het antimicrobieel gebruik kunnen met elkaar interfereren. Overleg en soms aanpassing van het beleid kan noodzakelijk zijn.
