Riassunto
L’assistenza polmonare appartiene ai più importanti provvedimenti di tutela dei pazienti in terapia intensiva, soprattutto durante il sostegno meccanico della respirazione. Nessun’altra assistenza è così importante ed influente sulla funzionalità degli organi vitali, come quella del distretto polmonare. La necessità di una tutela polmonare intensiva deriva dalle particolarità dell’intubazione endotracheale e della ventilazione meccanica. Il tubo sospende le funzioni del tratto respiratorio superiore, motivo per cui i gas respiratori non vengono più sufficientemente riscaldati, umidificati e filtrati. Il riflesso della tosse viene compromesso, per la presenza del tubo o per la somministrazione di analgesici e sedativi o completamente annullato, per impiego di miorilassanti. La produzione di secreti aumenta, a causa della ventilazione meccanica e per lo stimolo determinato dal tubo. La “clearance” mucociliare è ridotta.
