Riassunto

In questo capitolo discuterò la valutazione e la gestione dei pazienti violenti in acuto, affetti da un disturbo psicotico primario. La maggior parte degli studi su pazienti violenti ha preso in esame, dal punto di vista della diagnosi, campioni misti che includono una larga proporzione di soggetti con schizofrenia, rispecchiando la popolazione clinica. Analizzero con particolare attenzione gli studi di gruppi di adulti con schizofrenia pur prendendo in esame anche gli studi di gruppi misti; alcuni aspetti della valutazione e molti della gestione si applicano a tutti gli adulti con un grave disturbo mentale. Le diagnosi sono spesso imprecise, specialmente nei grandi studi comunitari, nei quali i pazienti sono spesso identificati attraverso i loro sintomi predominanti piuttosto che per la diagnosi. È assai scarsa la letteratura che tratti di violenza nei pazienti con disturbo delirante, ma questi soggetti sono simili a quelli con schizofrenia cronica, con importanti sintomi positivi e modesti sintomi negativi.
