Von einer Tachykardie spricht man bei Herzfrequenzen > 100/min im Ruhezustand. Als Hauptgruppen werden atriale Tachyarrhythmien, bei denen die Ventrikel nicht essentieller Bestandteil des arrhythmogenen Substrates sind („innocent bystander“) von ventrikulären Tachyarrhythmien unter-schieden. Ventrikuläre Arrhythmien sind durch ihr im Ventrikel gelegenes Arrhythmiesubstrat sowie durch die Unabhängigkeit der Arrhythmie vom Vorhofmyokard gekennzeichnet. Für atrioventrikuläre Tachykardien als dritte Hauptgruppe ist eine nomodrome atrioventrikuläre Leitung für die Aufrechterhaltung der Tachykardie erforderlich. Der klinisch gebräuchliche Begriff der supraventrikulären Tachykardie umfasst die Gruppe der atrialen Tachyarrhythmien sowie die atrioventrikulären Tachykardien. Die folgende Unterteilung innerhalb der drei Hauptgruppen berücksichtigt neben dem Mechanismus klinische, elektrokardiographische sowie elektrophysiologische Gesichtspunkte und hat zum Ziel, nach erfolgter Arrhythmieklassifikation direkt zu einem möglichst kausalen Therapiekonzept überzuleiten.
