Zusammenfassung
Die „Reverse-oblique-Fraktur“ (31A3 nach AO-Klassifikation) stellt eine seltene Form (5 %) der pertrochantären Femurfraktur dar. Aufgrund hoher biomechanischer Instabilität, meist schwerer Dislokation und der hohen Morbidität und Mortalität jeglicher konservativer Verfahren stellt die operative Therapie den Goldstandard dar. Aufgrund höherer biomechanischer Stabilität, geringerem zugangsbedingtem Weichteilschaden, reduziertem Risiko einer intraoperativen Frakturdislokation, geringerem Blutverlust, kürzerer OP-Zeit und Möglichkeit der schnelleren Rehabilitation erfolgt die Osteosynthese heute klassischerweise mittels zephalomedullären Verriegelungsmarknagelsystemen und optionaler Drahtcerclage. Die schwere Dislokation durch Muskelzug der pelvitrochantären- und Oberschenkelmuskulatur mit Weichteilinterposition erschwert die Reposition. Aufgrund der Flexions-, Abduktions- und Außenrotationsfehlstellung des proximalen Fragmentes ist die korrekte Platzierung des Marknageleintrittspunktes herausfordernd und sollte erst nach ggf. minimal-invasiver offener Reposition erfolgen. Inkorrekte Nagelplatzierung und Schenkelhalsschrauben/-klingenpositionierung (ventrale Lagen) und insuffiziente Reposition (zumeist residuale Varusfehlstellung) führen zu Komplikationen wie „Cut-out“, Schenkelhalsverkürzung/„Offset-Verlust“ und Pseudarthrose.
