Zusammenfassung
Insgesamt sind Femurkopffrakturen selten. Sie treten zumeist bei Luxationen der Hüfte im Rahmen von Hochrasanztraumen (80–90 % infolge „dashboard injuries“ bei Verkehrsunfällen) auf. Die Häufigkeit einer Femurkopffraktur im Rahmen einer Luxation wird zwischen 4–17 % beziffert. Die gängigste Klassifikation für die Einteilung von Hüftkopffrakturen ist die Pipkin-Klassifikation. Es stehen abhängig vom Verletzungstyp 4 Verfahren zur Auswahl. Erstens die konservative Behandlung, zweitens die Verschraubung des Fragments über einen zu wählenden Zugang, dann die Entfernung des Fragments oder eine totale Hüftendoprothese. Hier wird der Smith-Petersen als eine Möglichkeit zur Versorgung von Pipkin-Frakturen dargestellt. Grundsätzlich lassen sich alle Pipkinfrakturen auch über eine chirurgische Hüftluxation versorgen, die an anderer Stelle dargestellt wird.
