Zusammenfassung

Die Inzidenz von GallenwegskarzinomGallenwegskarzinomen steigt im höheren Lebensalter stark an, so dass sich auch bei über 70- und 80-jährigen Patienten immer öfter die Frage nach einer chirurgischen Therapie stellt. Allerdings erfordert dies meist eine große Leberoperation, so dass eine sorgfältige Abklärung obligat ist. Hierzu sollte neben kardiopulmonaler Diagnostik auch eine Leberfunktionsmessung mit Volumetrie gehören. Insbesondere bei eingeschränkter Funktionsreserve der Leber sollten im hohen Alter soweit wie möglich parenchymsparende Resektionen bzw. präkonditionierende Maßnahmen (z. B. Pfortaderembolisation) erwogen werden. Wenn sich keine Kontraindikationen ergeben, sollten auch geeignete alte Patienten reseziert werden, da sie in ähnlichem Maße wie junge Patienten von einer kurativen Operation profitieren.
