Zusammenfassung
Thrombozyten besitzen wichtige Funktionen bei Blutstillung, Gefäßreparatur und entzündlichen Prozessen. Im ersten Abschnitt dieses Kapitels wird auf die Physiologie und Pathophysiologie der Thrombozyten eingegangen. Thrombozytenfunktionsstörungen können sowohl erworben als auch angeboren sein. Bei den angeborenen Thrombozytenfunktionsstörungen werden Rezeptordefekte, Defekte des Zytoskeletts sowie Storage-Pool-Defekte der α- oder β-Granula unterschieden. Im darauffolgenden Abschnitt wird ein Überblick über die hereditären Thrombozytopenien gegeben, eine Gruppe seltener Erkrankungen sehr unterschiedlicher Pathophysiologie. Kriterien zur Diagnose sind die Unterscheidung zwischen syndromalen und nicht-syndromalen Formen, Thrombozytengröße, klinische Merkmalskombinationen und krankheitsspezifische Laborparameter. Die Immunthrombozytopenie ist eine Blutungskrankheit mit vorzeitigem Abbau und defekter Produktion von Thrombozyten. Sie ist eine Ausschlussdiagnose. Die Behandlung ist von der Erfahrung des Klinikers abhängig und orientiert sich an nationalen und internationalen Richtlinien. Thrombozytosen sind im Kindesalter im Allgemeinen ohne klinische Relevanz. Hiervon abzugrenzen sind bei Symptompersistenz über Monate und/oder Auftreten von Gefäßverschlüssen oder Blutungszeichen die sehr seltenen hereditären Thrombozytosen infolge genetischer Veränderungen des TPO- oder MPL-Rezeptors oder hämatologische Erkrankungen (Thrombozythämien).
