Hiatusbruch, Sphincterinsuffizienz, Refluxoesophagitis: Chirurgische Therapie

Summary

Reflux disease and its complications, rather than the mere presence of a hiatus liernia, are the indications for surgery. In the majority of clinically relevant cases an endobrachyoesophagus is present as well as the parietal sphincter on the terminal segment of the oesophagus being insufficient. Complications are secondary brachyoesophagus, stenoses and ulcers of the cardia, and malignant degeneration. Fundoplication acts as a real replacement for the insufficient sphincter. In the presence of acid hypersecretion combination with vagotomy is advocated.

Zusammenfassung

Die Refluxkrankheit und ihre Komplikationen, nicht die Hiatushernie, stellen die operative Indikation dar. In der Mehrzahl der klinisch wichtigen Fälle liegt nicht nur eine Insuffizienz des unteren Oesophagussphincters, sondern auch ein Endobrachyoesophagus vor. Komplikationen sind der sekundäre Brachyoesophagus, Stenosen und Ulcéra an der Epithelgrenze, maligne Entartung. Die Fundoplicatio wirkt als echter Ersatz des insuffizienten Sphincters. Bei Hypersekretion ist die Kombination mit Vagotomie von Vorteil.
