Zusammenfassung

Die Zahl ambulant durchgeführter operativer Eingriffe hat in den letzten Jahren ständig zugenommen. Diese Entwicklung wurde durch die Fortschritte der anästhesiologischen und chirurgischen Patientenversorgung ebenso gefördert wie durch das zunehmende Kostenbewußtsein. In dem Maße, in dem selbst größere Operationen ambulant durchgeführt werden sollen, nimmt auch die Bedeutung der postoperativen Schmerztherapie zu. Ein entscheidendes Entlassungskriterium nach ambulanten Eingriffen ist bekanntlich eine adäquate Schmerzkontrolle [166, 169]. Hinzu kommt, daß Schmerzen Übelkeit und Erbrechen auslösen können [6], die Entlassung verzögern und die Häufigkeit von nicht geplanten Krankenhauseinweisungen in die Höhe treiben [110]. Eine ausreichende Schmerzlinderung kann schließlich die Belastung der chirurgischen Wunde vermindern und dadurch Schwellungen, Hämatombildung oder Infektionen reduzieren [169].
