Zusammenfassung
In der Kindheit ist Schwindel ein selteneres Leitsymptom als bei Erwachsenen. Die meisten Schwindelformen und vestibulären Syndrome des Erwachsenen können sich jedoch ebenso in der Kindheit manifestieren, weshalb wir uns in diesem Kapitel auf wesentliche Punkte der richtungsgebenden Anamnese beschränken. Episodische Schwindelsyndrome in der Kindheit können durch Migräne bzw. ein Migräneäquivalent auftreten oder als epileptische Aura, bei Perilymphfisteln und selten im Rahmen einer familiären episodischen Ataxie. Dauerdrehschwindel kann Folge einer Labyrinthitis, Neuritis vestibularis oder eines Schädel-Hirn-Traumas sein. Auch der benigne periphere paroxysmale Lagerungsschwindel ist bei Kindern meist traumatisch bedingt. Oszillopsien bei Kopfbewegungen und Gleichgewichtsstörungen im Dunkeln sind typisch für die bilaterale Vestibulopathie, die bei Kindern z. B. nach bakteriellen Meningitiden oder durch ototoxische Antibiotika entsteht. Außerdem gibt es die bilaterale Vestibulopathie auch angeboren und zwar durch Labyrinthfehlbildungen (Tabelle 6.1). Die Behandlung der unterschiedlichen Schwindelformen entspricht der Erwachsener, sollte jedoch in enger Zusammenarbeit mit dem Pädiater erfolgen.
