Zusammenfassung
Mit Hilfe der Kontrolle der Plasmakonzentrationen von Arzneistoffen kann der Arzt die Effizienz einer Pharmakotherapie verbessern und die Toxizität von Medikamenten minimieren, indem er ein genaueres und präziseres Dosierungsschema durch die Kalkulation der individuellen pharmakokinetischen Parameter für den einzelnen Patienten ermittelt. Bei vielen Medikamenten existieren jedoch nur schwache Korrelationen zwischen der Plasmakonzentration und dem therapeutischen bzw. toxischen Effekt, sodass die Bestimmung von Plasmakonzentrationen nicht bei jedem Pharmakon von Nutzen ist. Deshalb ist es notwendig, die pharmakodynamischen und pharmakokinetischen Eigenschaften der Einzelmedikamente zu kennen um zu entscheiden, ob die Bestimmung von dessen Plasmakonzentration gerechtfertigt ist und um — wenn dem so ist — zu wissen, wie die Probengewinnung korrekt durchgeführt und wie die Konzentrationsmessung sinnvoll interpretiert wird [1]. Der unkritische Einsatz der Plasmakonzentrationsbestimmung führt nur zur unnützen Datensammlung und einer Verschwendung von Zeit und Geld [2]. Die Durchführung der Konzentrationsbestimmung von Arzneimitteln wird international mit dem Begriff „Therapeutisches Drug Monitoring“ (TDM) bezeichnet.
