Abstract
Von herkömmlicher Darstellung abweichend unternimmt es dieser Beitrag, die Mehrschichtigkeit der vesikorenalen Refluxkrankheit aufzugliedern, Hauptgesichtspunkt ist es, die einfache, nichtobstruktive, morbidativ überwiegende Refluxkrankheit von der kleineren Gruppe des obstruktiv überlagerten komplizierten Refluxes zu unterscheiden. Die 2. Gruppe hat den größeren Krankheitswert.
Die einfache Form, mono- wie bilateral, geht knospenbedingt zurück auf autochthon trigonale Dysmorphie samt ostialen Insuffizienzen und samt genetisch verbundener gradueller urorenaler Hypodysplasie.
Die komplizierte, klinisch in sich immer schwerwiegende Form besteht aus Koinzidenz der einfachen frühembryonal entstandenen Refluxkrankheit und hinterher aufgesetzter infravesikaler Obstruktion, diese effektiv erst im 2. Trimenon, weil das Allantoisventil vorher schützte.
Anstelle von zweizeitiger Koinzidenz zweier Entitaten kann frühembryonale Obstruktion alternativ urorenale Pan-Dysplasie verursachen, wozu dann bilaterale Refluxivität integral gehört und was vorzeitigen Verschluß des Allantoisgangs voraussetzt Das nur einseitig refluxive Klappensyndrom ist darin jedoch nicht unterzubringen. Es verweist stattdessen auf die vorerwähnte Koinzidenz zweier Entitaten.
Neben permanent infravesikaler Obstruktion gibt es wahrscheinlich auch fetal temporäre Obstruktionen mit genetisch bilateral-integraler Refluxivität. Das PBS und das Neck-Syndrom (Synonym: non neurogenic neurogenic bladder) passen hierher, beide ausgestattet mit ähnlicher urorenaler Dysplasie wie das refluxive Klappensyndrom.
Die renale Hypoplasie der einfachen Refluxkrankheit (Grad III und IV) geht auf quantitativ verminderte metanephronale Induktion zurück, eine fortgezeugte Beigabe der zugrundeliegenden imitativ entstandenen Schadhaftigkeit der Ureterknospe. Dysplasierte Refluxnephropathie (Grad V) geht zurück auf qualitativ verminderte Induktionskraft. Virulente aszen-dierte bakterielle Pyelonephritis überzieht postnatal beide Ausprägungen zusätzlich mit parenchymreduzierenden Narbenzonen und der Gefahr zu renovasculärer Hypertonie.
Während die Diagnostik der Refluxkrankheit kaum Probleme mit sich bringt, auch nicht die Modalitäten der beiden gegensätzlichen Therapien, sind die Indikationen zwischen invasiv und nicht invasiv nicht streng genug gezogen.
Unsere Darstellung der genetischen Mehrschichtigkeit der Refluxkrankheit erscheint zunehmend auch in der englischsprachigen Fachliteratur. Die deutschsprachige verweigert sich.
