Zusammenfassung
Metoprolol CR/ZK ist eine Arzneiform, die aus Hunderten von Metoprolol-Succi-nat-haltigen Pellets besteht, die von tablettenformenden Trägerstoffen umgeben sind. Jede Mikrokapsel funktioniert als eine eigenständige Einheit für die kontrollierte Freisetzung von Metoprolol. Bei jeder Mikrokapsel handelt es sich um eine Diffusionszelle, die entwickelt wurde, um Metoprolol-Succinat in nahezu gleichmäßiger Rate über einen 20-Stunden-Zeitraum freizusetzen, unabhängig von der Nahrungsaufnahme, pH-Wert und anderen physiologischen Variablen. Wenn man die Halbwertszeit von Metoprolol berücksichtigt, ergibt sich daraus eine gleichmäßige Plasmakonzentration über 24 Stunden hinweg bei einer Einnahme nur einmal täglich. Mit dieser Arzneiform lassen sich so die Fluktuationen der Plasmakonzentration des Arzneistoffes vermeiden. Dies führt zu einer gleichmäßigen β1-Blockade über 24 Stunden bei Dosierungen von 12,5 bis 200 mg einmal täglich. Die Plasmafluktuationen sind deutlich verringert im Vergleich zu den Arzneiformen, die den Wirkstoff sofort freisetzen, und dabei ist die Kardioselektivität sogar bei der 200-mg-Metoprolol-CR/Zok-Tablette gut erhalten. Die β1-Blockade wird für 24 Stunden aufrecht erhalten. Metoprolol CR/Zok ist gut verträglich; eine Verbesserung des Wohlbefindens durch Metoprolol CR/Zok hat sich an Patienten mit chronischer Herzinsuffizienz gezeigt, wie dies mit den Ergebnissen der MERIT-HF-Studie belegt wurde.
