Zusammenfassung
Es herrscht heute allgemein Übereinstimmung darüber, daß jede akute Aortendissektion, bei der die Aorta ascendens in die Dissektion einbezogen ist (De-Bakey-Typ-I, II, Typ-III bei retrograder Dissektion; Stanford-Typ-A), umgehend chirurgisch versorgt werden muß (5–8,12,14,15). Hier weisen vergleichende Untersuchungen eine eindeutige Priorität der chirurgischen Behandlung gegenüber der medikamentösen nach: Nach alleiniger medikamentöser Behandlung der Aszendensdissektion beträgt die Letalität ca. 70%, nach chirurgischer Behandlung ca. 30%. Anders ist die Situation bei der akuten Dissektion der Aorta descendens (De-Bakey-Typ-III; Stanford-Typ-B): Hier beträgt die Operationsletalität ca. 50%, die der alleinigen medikamentösen Therapie ca. 20% (11, 12, 16). Daher ist bei der akuten Deszendens-dissektion primär eine medikamentös-konservative Behandlung anzustreben (antihypertensive Therapie, beginnend auf der Intensivstation, später als Langzeitmedikation) (Tabelle 1). Zur operativen Intervention zwingen bei der akuten Deszendensdissektion folgende Situationen: 1. persistierende Schmerzen, 2. progressive Dilatation des „falschen“ Lumens, 3. Aortenruptur, 4. Ischämie eines vital wichtigen Organs.
