Zusammenfassung

Die Erweiterung von Koronararterienverengungen unter Verwendung spezieller Ballonkatheter, die perkutane transluminale Koronarangioplastie (PTCA), basiert auf der Arbeit von Dotter, der 1964 in Portland zusammen mit Judkins erstmalig ein peripheres Gefäß mit einem koaxialen Kathetersystem dilatiert hatte. Während seine Arbeit in den USA wenig Verbreitung fand, wurde sie in Europa von Zeitler weiterentwickelt. Grüntzig modifizierte und miniaturisierte das Kathetersystem durch Entwicklung eines dünnen Ballonkatheters, der über eine 8 F oder 9 F Schleuse eingeführt werden konnte. Mit diesem Verfahren wurden in Zürich und Frankfurt 1977 die ersten 6 therapeutischen Koronardilatationen durchgeführt. Vorversuche waren mit diesem Verfahren an Koronararterien im Tierversuch und an peripheren Arterien und Nierenarterien am Menschen sowie intraoperativ im Rahmen aortokoronarer Bypassoperationen in San Francisco und Zürich durchgeführt worden.
