Zusammenfassung
Die Sorge um die Sicherheit von Blutübertragungen hat erheblich zugenommen, seitdem man von der möglichen Übertragung von Viren weiß, die Krankheiten wie Hepatitis und AIDS (erworbenes Immundefektsyndrom) hervorrufen können. Die Gewinnung von autologem Blut ist eines der mit größtem Erfolg und mit zunehmender Häufigkeit eingesetzten Verfahren zur Verminderung einer allogenen Blutexposition bei elektiven chirurgischen Eingriffen. Üblicherweise werden im Rahmen eines präoperativen Eigenblutspendeprogramms vor einem geplanten chirurgischen Eingriff mehrere Phlebotomien in relativ kurzen Zeitabständen durchgeführt. Die Anzahl der benötigten Bluttransfusionseinheiten hängt im wesentlichen von Art und Umfang der Operation ab. Einschränkende Faktoren sind die begrenzte Lagerfähigkeit der Erythrozytenkonzentrate (5–6 Wochen) und das erythropoetische Potential des Patienten/Spenders. Abgesehen von der Verfügbarkeit von Eisen im Körper, hängt dieses erythropoetische Potential im hohen Maße von der Erythropoietinbildung im Körper ab, die sich im Falle autologer Blutspenden als sehr gering erwiesen hat [2, 10].
