Lokale Therapie des Rektumkarzinoms

Zusammenfassung

Bei der Entscheidung zwischen lokalen Verfahren und radikaler Resektion zur Behandlung des Rektumkarzinoms spielen bildgebende Verfahren eine bedeutende Rolle. Bereits die konventionelle Radiologie war in der Lage, sowohl den Tumorbefall des Rektums zu identifizieren (Abb. 1 a, b), als auch das Vorhandensein von Sekundärlokalisationen in der unmittelbaren Umgebung (Abb. 1 c) und in entfernt liegenden Strukturen (Abb. 1 d) zu verifizieren. Diese Fernmetastasen konnten aber nur in mit konventionellen Methoden leicht zugänglichen Organen (Lunge, Skelettsystem) gefunden werden. Zweifelsohne haben die neuen Imagingverfahren (Sonographie, CT und Magnetresonanz, [MRI]) das Staging der Tumorkrankheit des distalen Rektums perfektioniert [17].
