Zusammenfassung
Nicht nur zur Therapie des Endometriumkarzinoms, sondern auch zu seiner Diagnostik nimmt heute die Operation unbestritten den ersten Platz ein. Vor 20 Jahren wurden in unserer Klinik nur 36% aller Frauen mit einem Endometriumkarzinom hysterektomiert, heute sind es 84%, obwohl die Selektion ungünstiger Fälle zugenommen hat [16,17]. Das Durchschnittsalter stieg um über 5 Jahre und der Anteil der Patientinnen mit ausgedehnteren Stadien vermehrte sich von 15% auf über 30% [17]. Während früher noch diskutiert wurde [24], ob durch eine Kontaktbestrahlung nicht ähnliche Heilungsergebnisse wie durch die Operation zu erzielen wären, überwiegt heute der Gewinn bei weitem, den die Operation bringt durch die Entfernung unerkannter extrauteriner Metastasen und durch die Verhinderung eines lokalen Rezidivs im Uterus. Unabhängig davon steht heute die Operation auch zur Diagnostik im Mittelpunkt.
