Zusammenfassung

Technische Weiterentwicklungen der Langzeit-EKG-Systeme haben eine originalgetreue Wiedergabe des niederfrequenten ST-Segmentsignals ermöglicht. Dadurch können bei Patienten mit koronarer Herzkrankheit (KHK) passagere Myokardischämien im Langzeit-EKG erfaßt werden. Die visuelle Auswertung der Aufzeichnungen ist jedoch zeitaufwendig, und ihre Qualität hängt in hohem Maße von der Erfahrung und Konzentrationsfähigkeit des Auswerters ab. Ob dem Nachweis ischämietypischer Episoden im Langzeit-EKG bei unbekanntem Koronarbefund diagnostische oder prognostische Bedeutung zukommt, ist noch weitgehend ungeklärt. Aus diesen Gründen ist die Methode bisher nicht für die Routinediagnostik eingesetzt worden. Als Alternative zu der problematischen visuellen Auswertung bietet sich eine Trenddarstellung relativer ST-Streckenabweichungen von der Isoelektrischen an. Ziel der vorliegenden Arbeit war es, die Auswertung solcher ST-Streckentrends zu validieren und die klinische Bedeutung symptomatischer und asymptomatischer ST-Streckenabweichungen für verschiedene Patientengruppen zu analysieren.
