Zusammenfassung
Trotz beeindruckender Ergebnisse in der Therapie der terminalen Niereninsuffizienz durch Dialyse und Nierentransplantation hat in den prädialytischen Stadien der chronischen Niereninsuffizienz die konservative Therapie weiterhin eine entscheidende Bedeutung auch im Hinblick auf das Spätergebnis in der aktiven Behandlungsphase durch Dialyse und Transplantation. Die konservative Therapie ist besonders deshalb in den letzten Jahren wieder in den Mittelpunkt wissenschaftlicher Untersuchungen gerückt, da es klinische Hinweise für eine Verzögerung der Progredienz der chronischen Niereninsuffizienz durch die Eiweißrestriktion gibt [1, 2, 3, 15, 16, 19, 22, 25, 26, 29, 32, 36]. Bei den meisten klinischen Studien handelt es sich um retrospektive und unkontrollierte Untersuchungen. Aus diesem Grunde wird gegenwärtig eine prospektive und randomisierte europäische Multicenterstudie durchgeführt, urn an einem randomisierten Krankengut den Einfluß der Proteinrestriktion auf die Progredienz der chronischen Niereninsuffizienz zu prüfen. Für die klinische Praxis hat sich die Einteilung der chronischen Niereninsuffizienz in 4 Stadien (in Anlehnung an Sarre) bewahrt:

Stadium der vollen Kompensation (eingeschränkte Nierenleistungsbreite ohne Anstieg der harnpflichtigen Substanzen; glomeruläre Filtrationsrate zwischen 80 und 50 ml/min);

2.
Stadium der kompensierten Retention (Serumkreatinin zwischen 120 und 700 μmol/l; glomeruläre Filtrationsrate zwischen 50 und 10 ml/min);

3.
Stadium der dekompensierten Retention (Serumkreatinin über 700 μmol/l; glomeruläre Filtrationsrate zwischen 10 und 5 ml/min);

4.
Terminalstadium (Serumkreatinin über 900–1000 μmol/l; glomeruläre Filtrationsrate unter 5 ml/min).
