Chirurgische Behandlung des arteriosklerotischen Verschlusses im aortofemoralen Bereich

From: 1984 to 1990, 600 patients with aortofemoral occlusive disease were operated on at the Department of General and Vascular Surgery of Province Specialistic Hospital in Wroclaw. This group of operations consisted of 284 (47.3%) endarterectomies, 228 (38%) aortobifemoral prosthesis implantations, 62 (10.3%) aortounifemoral prosthesis implantations, 18 (3%) ilicofemoral and 4 (0.6%) aortoiliac prosthesis implantations. In cases of obstruction of superficial femoral artery and a poor profunda femoris outflow, the procedures were completed by profundaplasty in 61 patients (10.2%) or femoropopliteal bypass in 13 patients (2.2%).

The mean age of operated patients was 60 years. The cumulative limb salvage rate after 1 and 24 months was 98.3% and 90.5%, respectively.

Zusammenfassung

Im Zeitraum von 1984 bis 1990 wurden in der Abteilung für Allgemeine und Gefäßchirurgie des Spezialistischen Krankenhauses in Wroclaw 600 Kranke mit einem aortofemoralen Verschluß operiert. Bei den operativen Verfahren wurden folgende Eingriffe vorgenommen: die Endarteriektomie in 284 Fällen (47,3%), die Implantation einer bilateralen aortofemoralen Prothese in 228 (38%), die Implantation einer unilateralen aortofemoralen Prothese in 62 (10,3%), aortoiliakale Prothese in 4 Fällen (0,6%) und iliakofemorale Prothese bei 18 Patienten (3%). In den Fällen, bei denen intraoperativ ein Verschluß der Arteria femoralis superficialis festgestellt und der Rückfluß aus der Arteria profunda femoris als unzureichend beurteilt wurde, wurde die Operation bei 61 Patienten (10,2%) um eine Profundaplastik und bei 13 Patienten (2,2%) um einen femoropoplitealen Bypass ergänzt. Das Durchschnittsalter der operierten Patienten betrug 60 Jahre. Der kumulierte Bein-Erhaltungs-Index betrug nach 1 Monat 98,3% und nach 2 Jahren 90,5%.
