Porous Versus Non-Porous Synthetic Grafts — Personal Observations

Summary

According to their numerous positive aspects, Dacron grafts have been used in clinical practice from 1952 until now. However, there are two disadvantages of Dacron grafts: 1) extreme bleeding through the pores; and 2) high sensitivity to infection.

Progresses in technology have produced two forms of nonporous grafts: Dacron impregnated grafts with standard texture, and microporous PTFE grafts; the latter grafts have eliminated the problem of extreme bleeding through graft pores. The Dacron-impregnated grafts have given rise to new problems: early graft thrombosis, difficult handling compared to standard Dacron grafts, high price, etc..

Therefore, it cannot yet be stated that Dacron-impregnated grafts can completely emulate the advantages of PTFE grafts.

Zusammenfassung

In Anbetracht ihrer zahlreichen positiven Aspekte sind Dacronprothesen seit 1952 bis heute in routinemäßigem klinischem Gebrauch. Jedoch gibt es zwei Nachteile der Dacronprothesen: — extreme Blutung durch das Porengewebe, — hohe Empfindlichkeit gegenüber Infektionen. Die technische Weiterentwicklung auf dem Gebiet der Gefäßprothesen führte zu zwei Typen nichtporöser Grafts: imprägnierte Dacronprothesen mit Standardtextur und mikroporöse PTFE-Prothesen. Diese beseitigten das Problem der Blutung durch die poröse Textur. Die imprägnierten Dacronprothesen zeigten dabei neue Probleme: Neigung zur Frühthrombose, schwerere Handhabung im Vergleich zu den Standard-Dacronprothesen, hoher Preis u. a. Daher kann nicht gesagt werden, daß imprägnierte Dacronprothesen die Vorteile der PTFE-Grafts aufheben könnten.
